How It Works
Getting Started With IDD Adult Services
We talk with the participant about what they want to work on, together with the family and, when there is one, the support coordinator.
The assessment leads to an individual service plan listing goals, the staff support needed, and how progress gets measured.
Staff document progress at the center and review the plan with the participant and family, changing goals as skills grow.
Referrals
For Supports Coordinators and Case Managers
We accept referrals directly from supports coordinators and case managers, and we work with ODP waiver programs for adults with intellectual and developmental disabilities.
Call us and we will confirm capacity, arrange a tour for the participant and family, and schedule the intake assessment.

- Waiver program and authorized service, if known
- Goals the plan lists and the level of staff support needed
- Behavioral support, medical, and mobility considerations
- Transportation needs and the pickup address
- Contact details for the family and for you
